Appointments and Office Hours:

Patients are seen by appointment only. New patients are welcome and physician referrals are only required if patient's insurance require one.

Our office hours are Monday through Thursday 8:00 AM to 5:00 PM and Friday 8:00 AM to 12:00 PM.

Insurance

As a service to patients, our office will bill the patients insurance for services rendered. You are responsible for knowing what your policy coverage includes as well as what your co-pays and deductibles are. We will file both Primary and Secondary insurance policies. However, we require any applicable co-payments and/or deductibles be paid at time of service. We do not file cosmetic procedures. They are to be paid at time of service.

For patients who do not have insurance, we require payment in full at time of service. Payment may be made with cash, check, or credit card.
